- Policy contract
- Duly accomplished "Policy Loan Agreement" form (3 copies) signed by:
- Policyowner
- Irrevocable beneficiary/ ies
If beneficiaries are minors and equity of each is not more than 50k, require Affidavit of Parental Authority; if equity is over 50k, require Court Order.
- Assignee, if the policy is assigned
If assignee is a corporation, require Board Resolution or Secretary's Certificate giving executing officer authority to sign on behalf of the corporation
- Two (2) valid IDs of policyowner
If negotiated through a third party, require Authorization letter and two (2) valid IDs of representative in addition to the above
If policyowner is abroad, require Special Power of Attorney duly notarized and authenticated by the Philippine Consular Office where policyowner resides.
- Written request or duly accomplished "Dividend Withdrawal" form signed by:
- Policyowner
- Irrevocable beneficiary/ ies
If beneficiaries are minors and equity of each is not more than 50k, require Affidavit of Parental Authority; if equity is over 50k, require Court Order.
- Assignee, if the policy is assigned
If assignee is a corporation, require Board Resolution or Secretary's Certificate giving executing officer authority to sign on behalf of the corporation.
- Two (2) valid IDs of the policyowner.

If policyowner is abroad, require Special Power of Attorney duly notarized and authenticated by the Philippine Consular Office where policyowner resides
Policyholder may either accomplish the "Change of Address" form available at any of our offices; or,
Simply write a letter to Policyholder Services Division advising us of the change of address.
Note: Should a policy lapse, it may be reinstated if it is still within three (3) years from date of lapsation.
- Duly accomplished "Application for Reinstatement" form signed by:
- Policyowner and/ or payor
- Insured
- Payment of current premium
- Payment of all unpaid premium plus overdue interest an old loan interest (if any) at 12% per annum/
- Approval by the Company and classification of insurability.
If reinstatement is accompanied by a request for a policy change, refer to Annex A for additional requirements.

* The form should be accomplished by each beneficiary who is 18 years old or older.
Beneficiaries below 18 years old may be represented by the parent or guardian.
If the share of the minor beneficiary does not exceed P50k, require Affidavit of Care and Custody if represented not by parent but by guardian.
If the share of the minor beneficiary exceeds P50k, require:
- Guardian's Bond approved by competent court if represented by the parent; or
- Letters of Guardianship from competent court if represented not by parent but by guardian.
- Death Certificate issued by the Local Civil Registrar of the place of death, and bearing his official signature and seal.
- Birth/ baptismal certificates of the insured and beneficiary/ ies.
- Marriage contract if the beneficiary is the surviving spouse.
- Attending Physician's Statement - if insured died of illness, the doctor who last attended to the insured must be the one to accomplish this.
- Post mortem or autopsy report in case of accidental or violent death.
If the insured is a victim of a crime, copy of the Criminal Complaint filed with the Prosecutor's Office to put on record the nature of the insured's death.
